Notes
This version has the 4th & B'Way logo on spine and cassette shell. The cassette shell is opaque white with "100 • 0" underneath eleven short vertical hatchmarks under the timing window, all in recessed text. The printable area's top left and right corners are very slightly rounded regular corners, unlike the other versions. And in the bottom left and right corners of the shell are large circles with smaller circles inside of them. Along the top left edge of the cassette shell on each side is a small ∆ in a circle.
Program repeats on both sides.
