This has got to be mine, but I have:
side A runout: o XSM-58719-3L
side B runout: o XSM-58720-2G
I also swear it says "HI" in the runout opposite the matrix on side 1, but that doesn't appear to be stamped so it must just be an odd scratch. -

More info about Columbia inner sleeves. As you can see in this article, from 1963 - 1967 they used only a clear plastic inner sleeve, with a rounded bottom on it. -

The sleeve in question in the release photos here is circa 1968. The inner sleeve pictured is wrong- as one other person commented, it shows albums released as late as 1968/69. In fact, all Columbia releases between 1963 and 1967 came with a clear plastic inner sleeve only. -
