My PT/RECORDINGS copy arrived the other day. I bought it here on Discogs. A wonderful recording, clean, flat, quiet. I am enjoying it. The inner sleeves are super tight as others have mentioned. It's because the card stock used for them is so thick. I've replaced them with simpler Mo-Fi inner sleeves for easy in/out. I store the OG ones along with its gatefold, within the outer sleeve. As far as I can see on the Discogs database, the only current reissue is a 2015 CD. So this 2011 LP version appears to be the most current. To answer the question, the inner sleeve Issue isn't resolved.

My copy was absolutely coated in fine paper 'dust' fresh out of it's shrink-wrap - weird, normally these K-scope releases are better than that. I didn't seem to have any issues with tight inner sleeves as others have mentioned though.
Fantastic album too! - even more so considering this is a collection of outtakes and b-sides! -
