Does anyone know where the “stage” cover image came from ?

Question on the numbering. I've only ever seen these numbered /50... and the 4 different covers. Supposedly 200 sold in London and 100 in Berlin. But how did we end up all of them being x/50? The math doesn't work. Are there duplicate numbers then? Or unnumbered ones (which I've never seen on any of the dozens sold on ebay over the years).

*edit - just noticed the notes say "Each of the first 200 released are numbered x / 50." but I've never seen any unnumbered ones?
