My CD booklet has a spade shaped hole punched through the 1st 3 pages of the booklet. The hole is on the upper right on the last ankle on the upper right corner of the cover. The hole appear very clean cut. Doesn't look like it has damaged the 1st 3 pages at all. Is this common ?
