Very similar to Highway 61 Revisited, with the same runout matrix numbers and same sleeve details. The only difference is that this version has the erroneous '62575' label matrix. There is at least one other version with this incorrect label matrix but that/those version(s) have completely different runout matrix numbers and/or no black box around the track-listing.

Gloss-laminated full-color cover with 3 laminated and overlapped glue-tabs onto un-laminated back cover (aka 'flipback') that has black & white halftone and black text. black box around tracks.
Track A4: On sleeve as "From A Buick Six" and on labels as "From A Buick 6" (Which is latter the most common title)
"CBS" monogrammed 'eye' logo on both cover front & back - and on labels.

I have a very similar version my runout matrix is 62572 - 2A - 1 and 2B on back. The label has a rough textured 7.5 cm inner circle and a little over 1 cm outer circle that is smooth. Have i got a different version completely? What do i do?
